New Patients

New patient appointments require credit card confirmation. Please fill out both the scheduling and the credit card forms below to confirm your appointment. New patient appointments made without credit card confirmation will not be held.

Once we receive your scheduling request and credit card confirmation, we will send you an email invitation to fill out new patient paperwork and consent forms. Please check your spam folder if you do not receive the invitation. If you are not able to submit the forms, please arrive 15 minutes before your appointment to complete the forms.

Existing Patients

Please choose “Existing Patient” under Type of Visit and complete only the scheduling form. We already have your credit card on file. You don’t need to enter it again.

Appointment Changes and Cancellations

Please note that appointment changes and cancellations cannot be completed online. Please call us at 415-445-9388 to reschedule or cancel your appointment.

We have a 24-hour cancellation policy; cancellations within 24 hrs. are subject to a $120 fee which will be charged to the card used to hold the appointment.
